Introduction:
At Self Physiotherapy, our aim is to deliver a professional, evidence based and individualised pelvic and reproductive physiotherapy service. The following information is here to help you understand how we operate our clinic.
The following information/agreement is between the patient (referred to as ‘patient’, ‘you’, ‘your’ and ‘client’) and Self Physiotherapy (referred to as ‘Self Physiotherapy’, ‘we’ or ‘us’)
Please read the following information carefully.
cancellation policy:
Life has no plan, and regardless of your situation there are times it is “out of your hands”. We ask you please provide 24-hours notice to cancel or reschedule your appointment to avoid a cancellation fee. A cancellation fee applies to cancellations or rescheduled appointments within the 24-hour window, or if you fail to attend your appointment. Our cancellation fee is 50% of the booked appointment cost.
This cost is not claimable by your private health provider.
In the event of physiotherapist sickness, or unplanned circumstance, a scheduled session may be cancelled. If this were to happen, we will reschedule your appointment as soon as possible. There will be no charge if your session is cancelled by us.
Payment:
Payment for your appointment is due on the day of your appointment with Admin. We accept card payments and, on request, bank transfer. Should your private health card not work on the day, a payment in full will be needed and you will receive an invoice allowing you to claim your private health rebate manually.
If you are late to your appointment, the appointment will begin at your arrival and finish at the scheduled end time. With consent and pending availability, if you prefer to have the full time allocated to your appointment, you may be charged for the extra time we are spend.
Our clinic does not tolerate discrimination, aggression or bullying. If we consider your manner to be dangerous, inappropriate or unsafe, we may stop the session and discontinue it. You will still be liable to pay the fee.
Informed Choice and Consent:
The client’s electronic signature & tick on the ‘Client Information form’ electronically confirms the client has read and understood the terms and conditions lined out on this page and in our ‘Privacy Policy’.
Assessments and examinations are carried out in a private and secure environment. Detailed questions will be asked of your concern and medical history, and an internal examination may be required with your consent. If you feel uncomfortable about any form of examination, you have the right to bring a support person with you.
Due to the nature of your concern, there may be personal questions asked. You have the right to refuse discussing topics if you feel uncomfortable.
You have the right to verbally refuse any treatment or assessment. You may withdraw your consent at any stage. As per guidelines and recommendations, you will be provided with the appropriate risks, benefits and alternative options to your assessment and treatments in order to make informed decisions for the management of your presenting concern. Clients under the age of 16 years old must be accompanied by a legal guardian, or parent, for pelvic floor examinations.
